i'd set it up now because guests will want to purchase gifts for you during the holiday season and it would be cool for them to get registry items.
Probably not more than 6 months in advance. I set up my registrys in July - getting married in December - and I'm already running into problems with stuff getting discontinued or out-of-stock.
If you have specific store you know you want to register at, just call and ask what they recommend.
Wellllllll a girl I grew up with just engaged about a week ago and she's already set up her registry at Target...
