okay why was i looking for that dress for months. i never came close to finding it. but possibly you can get a sheeth and have a bolero made for it? perhaps on etsy or something like that? maybe check out those etsy sellers that make wedding dresses they may be able to replicate it...
It's times like this I wish I had a professional stylist. They'd be able to find it. Since my party is in February, I don't think I have time to have the dress made. But, doesn't hurt trying. Thanks crebre!

It is an Elie Saab. Ready to wear is available at Neiman Marcus, Bergdorf Goodman and Saks. That dress may be haute couture. however.

Jenny Yoo makes a dress somewhat like that - less detail around the waist, and a little shorter, but with a similar shoulder detail.
any reasonably competent seamstress can make a similar dress by using pattern pieces from different patterns i.e a sleeveless dress pattern, then add the looser elbow length sleeve on one side etc.

Or a replica of this dress. I've liked this dress for a while now. A friend showed it to me about 6 months ago and I've had secret dreams of it ever since. Well...
