Well I got a custom gown made but I went with Gioavonni's Bridal Boutique and what I got did NOT made me happy! In fact I am still pretty POed about the whole thing. I would recommend working with someone local and get recommendations first.

I got my dress from Milly Bridal in China. My dress was not custom, it was part of their existing designs, but a lot of brides go the custom route and everyone I've spoken to has been pleased with the quality and the price.
What's your idea of an OK price range? I think most ladies can get their gown made by a seamstress for like $1500. That's too much for me, but that's not necessarily a bad price dress-wise. You could scale it back depending on what your dress has: lace, beads, crystals, etc. What are your particularities? If that's a word =]
check Etsy for custom dresses in all price ranges. You can search through dresses that sellers have already posted, or go to the Alchemy (custom) section and post what you want to have people bid on doing the custom work for you. Sooo fun!
I had a custom made dress from Casablanca because of my petite size. It cost me about $1000. You can google Casablanca and find a local bridal shop near you that can have the dress altered completely to your size to fit you.

Etsy vendor, BeyondTheReflection is making my custom gown for $375. This includes all material, labor, fittings, and alterations. I have just started this process with her, but so far so good.

I am using a person I found on Esty.com, CiCiBridal's Shop. She has been great to work with. She worked with me to combine two dresses I liked into one. One dress was her design and the other was one I found online.
She does a sketch of the dress for you so you can see it, that was $25.00. I fell in love with the sketch so I ordered the dress. My dress is $510 with shipping. She requires about 3 -4 months for the process. I should be getting my dress in mid-June for my mid-July wedding. She is located in Indiana.
