we went with the same company that did our STDs and invites so they would all match. they carry the whole suite (table numbers, place cards, thank you notes, programs, signs, etc.) for all their designs
vista print is another good option since they're really cost effective, but i didnt care for their designs.
I made my own and they turned out really nice. The templates were from Etsy - Two Yellow Shoes. I put one one a deep pink border on a black placecard (paper source) and another pattern on a lavendar border on a black placecard. They were some work but SO much nicer than everything else I've seen.
Try etsy.com -- a lot of people will be able to do them relatively cheap and how you want them!
We didn't have a whole lot of DIY at our wedding but I did make the menu/place cards. I purchased the materials from Michaels and they came out beautifully.
