You can't put your married name on it until you are officially married, so I would do your maiden name. As long as you change within the first year, you don't have to pay to get it changed
You should use your maiden name until you're married and have changed your SS card and driver's license.
And, I'm sure you have already also thought of this, but make sure that all of your honeymoon travel documents, i.e. airline tickets, etc. are in your maiden name -- EXACTLY as it appears on your passport.
I just did the same thing last week and the lady at the passport place told me to put my madian name because its my name now that I'm applying.

This may sound like a silly question, but I really have no idea. I am (right at this moment) in class getting ready to order my passport online and I don't know which last name to put on it. Do I use my maiden name or my married name?
