If you want them to.  We bought them for parents, grandparents, sister of the groom (she’s not a bridesmaid) and the officiant (who is a friend of ours).

@Happy7:  Our grandparents will get them.. Anybody you really want to ‘honor’ .. so along with all the ‘normals’ 2 of my dear friends (male) will be getting one too

@Happy7:  we gave them to our parents, grandparents, the groom and groomsmen, ring bearer, guest book attendant, ushers, and the cake cutters at our reception., but have also seen them given to those who hand out programs, those that read or sing in the wedding as well.

We got them for our parents, grandparents and MC’s (who are FI’s cousins).

Definitely grandparents. Grandmas tend to get their feelings hurt if they feel at all overlooked on the wedding day.
