We're getting our cake from Walker Cake Co out of Corning. I don't know if they would really be considered an old mom and pop place. The owner is fairly young and I think they just opened in the Corning area. But they do amazing cakes!
Say Cheesecake has amazing wedding cakes (not just cheesecake). They may have changed their name, but they are on Sheridan Drive in Buffalo. Also, Muscoreil's in North Tonawanda- my friend used them and ended up getting her sister's cake there too- it was sooo yummy! There is also Dessert Deli in Williamsville, a little pricey but well worth it! Good luck!

We're in Cleveland, so I can't run around to bakeries on a whim. I'll need to set things up in advance. Suggestions? Hometown, 50 year old type places...I've browsed the high-end cakeries but I'd rather go with the stand by mom-n-pop shop, if you know what I mean.
Related note: how much to expect to pay for vin-chet? standard 2-3 tier, buttercream, minimal decoration...
