I like the madlib idea.  I’m with bluebutterfly I’m confused on the second Will be there but without bells.

I get be there with bells on that is just a cute saying. But I’m not sure if you want to use it on all 3 responses.

I think that the wording is rather informal. I have a degree in English Lit, so maybe it just bothers me because I’m used to using formal English for things like invitations. If I were editing your invitations, I would get rid of all mention of bells. I think it will confuse most people. I am also wondering why you decided to say “Dad and Mom invite you…” This is extremely informal. Are you talking about your Dad and Mom? Your fiance’s Dad and Mom? You should really think about changing it to “Mr. & Mrs. X invite you…”

I really like your invite design!

I like the mad-lib idea a lot, but I was confused when I first saw the numbered list on the front of the card.  Have you considered using your image from your invitation and making the front of the card a little more simple, then on the mad lib put (name) (noun) etc. under the blank lines rather than the numbers?  I think that make it more apparent to guests what they’re supposed to do.

“Will be there with bells on” is cute, but I wouldn’t use the second and third option.  “Won’t be able to make it” or something similar seems like a more appropriate second option.

Also, and this may sound silly, you shouldn’t assume your guests know what omnivore/herbivore mean.

Is it really going to say “Dad & Mom,” or was that edited for privacy?

I think she said Dad and Mom because she didn’t want to put her personal info.  I agree that maybe it is a tad bit informal…  I like the will be there with bells on and get the quirky sarcasm of will be there without bells as well…

I love the invitation. I think the RSVP card has too much going on… I would pick which one you like more, the Madlib idea, or the Bells idea… with both its just overkill…
