I would do Ms. for your cousin if she is going through a divorce. Or, if you are worried about hurting her feelings, maybe just put her first and last name without a title?

For your FI’s widowed grandma, definitely Mrs.

It’s never wrong to address an adult woman as Ms. unless you know she does not like to be referred to in this manner.  An additional bonus:  When using the “Ms.” honorific, you can use her first name rather than her (soon to be) ex husband’s. 

I addressed invitations to widows in the form of “Mrs. John Doe.”  However, on their escort cards, I used the format of “Ms. Jane Doe.”
