Most properly,

on the outside of the envelope:

Mr Uncle Lastname
Street Address &tc

No need to advertise to the letter-carrier, or to the busybodies that share the apartment building lobby and read the envelopes that are two thick to fit in the letter-slots, who is shacking up with whom and what their minor children are called. The outer envelope is just supposed to get the invitation through the postal system to the right place

On the write-in line of the invitation, or on the belly-band or inner envelope if you don’t have a write-in line:

Mr Lastname
Ms Girlfriendlast
Miss Lastname, Miss Firstname2

In correct social form, the surname and firstname are not used together. That form is for business only; and etiquette eschews mixing up business with social events. If the children are boys, they typically get “Master Firstname1, Master Firstname2”. It gets more complicated in extended families, but if you get it right your gread-grandaunties will be terribly impressed.
