I haven’t worn the dress, but altering it that much is pretty major and not going to be cheap.  My friend bought a size 10 sample and had it altered to a 2, and the seamstress basically had to take the entire dress apart, re-cut it, and put it back together.  It cost as much as the dress!

I would look for the dress closer to your size at another store or online.

Are you saying that you are a street size 4?  I don’t know about Alfred Angelo but I’m normally a 4 or 6 and my dress had to be ordered in a size 10 so it might not be that off size wise.
