She can get a new, official birth certificate.  Each state has it’s own rules about this.

Actually, I looked it up.  For minors under 16, you don’t need a birth certificate. She can use the old passport as long as it’s not damaged.

She can go to the Department of Health and they will give her an original sealed copy of her birth certificate. I got mine in a matter of 15 min. I lost my original as well and needed it to apply for my passport. Hope this helps.

Getting a passport was a lot easier than I thought it would be. I think for minors if the parents can prove they are citizens then they let the children have a passport. I wouldn’t stress too much about it.

When do you need it by though? That might be the only problem, because It really did take 4 weeks to get mine. You can apply for it online and then mail in the papers with a picture.

You can pay to have them expedited.  Just to be clear – is this a US passport and you’re in Egypt?  If it is, I’d go to the embassy or consulate, explain the situation, and have one of the State Department consular officials explain what exactly is needed.

@Mrsgurzakovic:  Definitely.  Can you call them?  You’re more likely to get ahold of someone if you call, rather than being lost in the internet blackholes.
