@ShannonO84:  If your mom doesn’t care, and your dad does, then I’d do what makes your dad happy (as long as you like it too).

Both of my parents cared, so we went generic “Together with their families.”

I’ve seen lots of divorced parents on the invite with different last names.

No, your mom should not be included if she is not hosting, and your dad is absolutely right that he should be listed as the host. If, however, he is okay with it, you could include your mom out of respect. In which case, you write (without the word “and” and single-spaces between dad and mom)

Ms.Mrs. Mom-First Middle Last

request the pleasure of yoru company (not at church) or honor of your presence (at church)

etc.

@BuffaloDots:  There should be no “and” as that indicates that the two people are married. Each name goes on a separate line with no “and” between.
