ours isnt exactly a chart but were doing a ribbon board

and they will be propped up on little easels on the table like this (but they will be painted black

@piglet – Thank you!  An Etsy dealer actually created it.  We had it printed and then framed it ourselves.

I made mine in Photoshop, had it printed by uprinting.com (they offered the fastest turn around time) and then slipped it into a free-standing sign my mom had from her business.
