Hi there! Yes they do make major changes like those. A couple of years ago I worked in a bridal shop that sold Casablanca and I did several completely custom gowns for brides through them. (I even went through their Casablanca certification program.) You will need to reference one of their styles that has the a-line skirt shape you are looking for and they can use the design details from the fit and flair. cypresstree85 :  They can pretty much do anything you want. I had one bride combine elements and cuts from 4 of their dresses and then did other modifications on top of that. It came out so gorgeous in the end! Just remember that every change comes at a cost and the less changes you make, the less risk invovled. Are you good at visualizing things? I would recommend making another appointment at the store where you saw the gown you loved. Meet with your stylist and explain your vision. If the stylist is well-trained and experienced with customizations, he/she should be able to guide you through the process and help you find the right styles to reference.
