If you’re talking about the round ones:  Get a “circle punch” from the craft store, about 1.5 to 2 inches, and some cardstock 8×11 paper of the color you want.  Use regular paper to test it out until you’re ready for the final printing.  If you have Word or some other program, use the Circle tool in your software to make a circle smaller than your punch.  Make a few circles and test them out for right size using the regular printer paper so you don’t waste the good paper.  When you have the circle size you like (you can make the border be dots, dashes, plain, etc) and that works with your punch size, then just copy the circle a bunch of times, types the names/tables inside each circle (center them, nice font, etc), then print them twice on the good paper.  Use the circle punch to punch them out, put the toothpick between the two circles and a piece of strong double-stick tape to stick them together.  Should be easy once you figure out the circle size on paper that fits the punch size.

You could try those printable stickers from staples? Then you can jsut send them through your printer and have the writing on them. Then maybe do like PP said and get a circle punch so you have circles to put the stickers on! Good luck!

EDIT: Or maybe you could put a blank sticker on the back and the printed on on the front to stick them to the toothpicks!
