I am going through sperm issues too.  Similar to you they didn’t want to do much until they had two samples to compare.  Our second sample was somewhat better but not wildly different.  I believe the second concentration was around 14 million with only 1% morphology and close to 50% motility but very low percentage actually swimming in a nice straight like, maybe 3%?  We are in the process of investigating possible treatment to get the numbers up but i understand it’s possible we could get pregnant naturally bc it only takes 1!  But not super likely.  DH is having an ultrasound to see if he has varicocele and then we will consider surgery but my understanding is fixing it only might improve the sperm numbers.  But we are kind of borderline candidates right now for IUI or natural pregnancy and unless we get the sperm numbers up we might skip IUI and go straight to IVF.  But if we could get a bit of improvement it might be just what we need to avoid IVF.

Adding – we are doing the ultrasound bc the urologist thought he felt the varicocele in the physical exam he did.  And I’m pretty impatient waiting through all of this urology stuff for something that only might help so if the urologist doesn’t think the ultrasound is the right route for you don’t despair.  It isn’t a route to a quick magical fix.

omglauren :  Good update! It sounds like once he gets his blood work back and if horomones are good, he will send you guys to a fertility clinic. If his homornoes are off he will have him take medicine. What was the count of sperm for your DH? Pretty much same thing happeend with my Darling Husband. Due to how little sperm he produces (less than 10k), we have to go through to IVF. A fertility doctor will tell you if IUI or IVF is right for you.

omglauren :  Glad to see an update. Your last report sounded weird, but the urologist will give a more detailed report. An ultrasound would be prompted by the physical exam. If nothing came up during the physical exam, then there’s probably no need for an ultrasound. It sounds like your doctor is following the correct course of action. Hopefully, the second SA report shows lots of sperm!!

omglauren :  my husband needed an ultrasound following less than stellar SA. He said it was unpleasant but survivable lol. How is his overall volume? After lots of investigation our urologist finally had Darling Husband empty his bladder, give his sample, and then immediately provide a urine sample after he ejaculated. That when we found out all the little buggers were hiding in his bladder! Technical diagnosis is retrograde ejaculation. From there it was a clear move to IVF with ICSI and we were thankful to have an answer and a plan.

Scarlett11 :  my husband also has a varicocele and the urologist suggested waiting to fix it until after I was pregnant via IVF because it’s a longish recovery and wasn’t a guaranteed fix. That may be because he has 2 separate issues at play though…

omglauren :  I’m glad the results were better than you were expecting! Best of luck with the repeat SA!

Scarlett11 :  Darling Husband had varioceles that were removed via surgery. It did improve his sperm count and quality but his morphology remained very low and we didn’t have any luck following the surgery. We ended up going to IVF and got pregnant on our second round. I think maybe we should have gone straight to IVF since that whole surgery/recovery/TTC post recovery process took about 10 months!

LilliV :   hollyberry4 :  Thanks for the feedback ladies.  I think I need to get a bit tougher with the urologist and RE at our next appointments because that is kind of the same thing I have been reading online and at 35 I’m feeling anxious that my chances of having a child are slipping away waiting out all of this analysis of the varicolele issue.  I also feel like I’m getting kind of bad information on whether IUI is a real possibility for us with 0% – 1% morphology and very low forward motion motility.

omglauren :  Sorry for partially hijacking your thread.  Wishing you best of luck and will keep following your journey.

Scarlett11 :  Did your Darling Husband get a physical exam or have blood work done? Def investigate more before jumping to IVF. Its positive if you doctor said you could get pregnant naturally.

Scarlett11 :  as PP said get more testing for him, but once our doctors told us “this ain’t happening without IVF and ICSI” and we had one failed IUI we dove right into IVF and never looked back. And we weren’t running up against that much of a time issue (we’re both 32 now and I’m due in July) for the first kid, but we want more so I wanted to get started already. For us it took on the first shot – my body knew what to do once it got a fertilized egg, we just couldn’t get the sperm where it needed to be!  For IVF w/ ICSI you literally only need ONE good sperm per egg collected (for us that was 17), for IUI or natural conception you need a millions of good ones to increase your chances.

LilliV :  Your post is giving me so much hope! I am in the exact same situation : we’ve had 2 really bad SA results, but all my testing (hormones, antral follicule count, sonograms) came back perfect so we were told this week that our only shot would be IVF with ICSI. I am now just waiting on CD1 (should be in less than 3 weeks) to get the ball rolling!

niki1983 :  Good luck! It is so nice to have an explanation and a game plan. I hope you get a first try baby too!

LilliV :  Thank you! I really hope I do too! Were you able to freeze embryos to try for a second baby down the road?

niki1983 :  Yes! We collected 17 eggs, 11 successfully fertilized, and 4 made it to day 5 blast stage. We put one in and it took so we have 3 left in the freezer. We always said we want anywhere from 2 to 4 kids so we plan on using all of the embryos (while realizing that the chance of ALL of them taking is slim). So long as we get at least 1 more baby out of the 3 frozen embryos we’ll be done and not do another retrieval. If all 3 fail we will do one more round.
