I think you’ll be fine.  People can always “google” the address.  I don’t think its worth reordering.

I wouldn’t re-order. If any one is unsure about the city, they will just ask.

For most people, I wouldn’t worry.  But then, I have a coworker who swears Boston is in NY because there is a sign in NY for Boston (on the interstate), and that Las Vegas is in California, because it’s on the Pacific Ocean.  Never can understand some people.
