I didn’t do anything differently for my Maid/Matron of Honor. She stood out because she stood closest to me during the ceremony and gave a speech at the reception, that’s it. I wouldn’t touch the dress.

I second the idea for a different bouquet. I was going to have my Maid/Matron of Honor in a different color of the same dress, but we decided just to put everybody in the same thing. I think it looks a bit better. You can always differentiate between flowers, a sash like you were thinking, or even a nice brooch. 

If you do go with a cream sash I think that would look fine with cream colored flowers. The only thing I have to ask is: have you tried putting a sash around that dress in person? If the sash does not look right with the rouching in the middle of the dress that may be of more concern. 

Also maybe each of the girls could wear a flower in their hair and hers could be different?

In a lot of the weddings I’ve been to, the Maid/Matron of Honor wore the same dress but carried a slightly different bouquet. 

I get that the Maid/Matron of Honor wants to look different, but ultimately, I think she’s more distinguishable by her duties during the wedding–she’s usually the last one down the aisle (before flowergirls), the one standing next to the bride that holds the brides’ bouquet, she gets introduced at the reception as “MOH,” AND she usually gives a speech. 

You can remind her of these things if you really want to keep the wedding party looking all the same (although what’s a little sash or a few more roses, in the end).

For my Maid/Matron of Honor I am gonna do a slightly different bouquet with a different wrap around it. That is a good idea without modifying the dress
