I came across this idea a while ago about having your guests fill out a questionnaire about you as an activity/guestbook alternative. Below is my first attempt to create one and then we would print it on color paper ( it is still a little rough) and either place it on a table near the entrance or maybe even on the guests chairs. Please let me know what you think and give me any feedback on ways to improve the presentation to the guests and any other questions that would work better.

I’d ditch the marriage advice section; it gets really redundant. If you really want to do this, I’d leave it on a table as your actual guest book so people don’t forget to fill them out. Leaving them on the tables leaves them open to being thrown out during clean-up, or getting spilled on!

@anghp: What about maybe a life lessons or wisdom section or something? Where they could tell us one thing of use for our life they have learned like ” stop to smell the roses” , ” take chances” stuff like that. Do you think that would be better?
