We moved to NY for my husband to go to law school and have had a temporary address here for a few years. We just found out were staying here permanently because DH has accepted a job for after he finishes school. So now I have to go down to the DMV and transfer my license. I did my research and I know I have to have my SS card, my out of state license, and then it says proof of name and proof of date of birth. I cant find a list of acceptable documents for these ANYWHERE!! I dont want to get down there and have to go back because I brought the wrong things!! Does anyone know what I should take for proof of name and DOB? I have my marriage license which will probably work for one or both, but I have no idea really. Do I have to take my birth certificate? HELP!

@emilygrace07: Proof of name would be a wedding license and proof of birth would be a birth certificate. I think a birth certificate will also act as proof of name since it would have your maiden (Or correct if you didn’t change your name) name on it.

I moved out of NY due to fiancee’s job. Needed same documents here: SS card, old license, birth certificate. And I am aware marriage license serves as proof of name change.

@emilygrace07: be careful with the DMV. in Maryland, my marriage licence wasn’t sufficient, though it worked fine for SSA. i had to go back to the courthouse to get a certified copy with the raised seal.
call them to verify what documents you need.

I would also call the nearest one located near you and confirm if what you have will be approved prior to going.

@ajillity81: mine is a certified copied signed by a judge so I should be fine. I just need to get my parents to mail me my birth certificate.
@Bella83: I think youre right because I used my marriage license to get a new SS card with my new name on it.

@emilygrace07: I just changed to a NY drivers license. I used: old NJ drivers license, birth certificate, debit card (with NY address), and health insurance card. I also brought last year’s tax return just in case.
