How costly/complicated would it be to alter this long-sleeved lace gown?

I’m your size and had a lace dress with short sleeves and an open mesh back. It required a LOT of alterations, not gonna lie. I’ll start with the hemming: they take off the bottom part, then hem, then reattach the bottom so the lace still lines up. That keeps the nice scalloped edge and I imagine the same would be done for your sleeves. 
my biggest issue was that the waist needed to be taken in and up. That in turn meant the shoulders were too high.  basically the entire shoulders were removed, altered, and resewn. Taking away material from the shoulder area also helped with my issue (and your issue) of too much fabric in the back. Your dress waist looks like it hits at your natural waist though so that might not be a concern.  My dress was more structured than yours- I’m not sure if that makes it easier or harder! I do know that the bust was still too large and my seamstress said there was really no way to make it smaller. We just added padded cups and it worked given the structure of my dress. 
I hope that explanation helps- it’s kind of hard to describe! My alterations were roughly $800

Well it’s a beautiful graceful dress and suits your slender frame SO well. The sample you have on is obviously miles too big and clipped to the max . If the size you order is much smaller, perhaps all will be well, but l can’t see that this is going to be cheap as you will need to have those lovely sleeves restructured, and hemming is not just a matter of chopping.

If it’s the one I would get it! I ordered my 0 dress and according to the measurements it looked like it would need to be altered. When I  got it it fit like a glove. Obviously had to be hemmed but that’s pretty standard. I would order it and go from there.
