If someone goes to the store, prints out your registry, picks out something that’s not listed, and when they go to pay, your registry code is scanned, no matter what they buy (if it’s on your list or not) it will be added to your registry at the time of purchase because it was purchased for you.

Example: We registed for our everyday plates in box sets which gave us 10 place settings. They were listed as fulfilled. My mother thought we needed 12 for big family dinners, and bought us an addition 2 place settings that we never had listed on our registry, but they are on there now as fulfilled because our registry code was scanned when she bought them along with the original set of 10.
