I guess you could describe the stones as centre stone with 3 sides stones in triangle on either side, but I agree with PP about just showing this picture to the jeweler, otherwise I don’t know if you’re going to be able to convey exactly what you want. 

ETA: if you don’t want to go to the jeweler to show him the pic, see if he has an email address you could send it to
