Mine were pretty standard but here you go:

TWO THOUSAND ELEVEN

dinner, dancing & merriment to follow

RESPONSE CARD:

THE FAVOR OF YOUR REPLY IS REQUESTED

Hope that helps!  For the other cards you could include directions an information about hotel room blocks, and maybe a reference to your wedding website for more information.

Just imagine the text neatly spaced out with blanks lines and some decoration in between. As for the invitation wording: my parents have different last names, so we listed them separately (and did the same for the groom’s parents though they share a name) so everyone will know the correct names.

INVITATION:

[Bride’s mother]

request the pleasure of your company

at the marriage of their daughter

[Bride] 

to

[Groom]

son of [Groom’s father] and

[Groom’s mother]

I think the other two cards that came with you pocketfold are usually used for a separate reception card (when the main invite is used only for the ceremony), map, accomodations info, other general info, homepage URL etc.
