MemberAre you including a directions card? If not, then you need to put the address. Zipcode is not needed.
Memberpersonally, im a fan of just the church name. I just like the look of an invitation better when they are more clean and simple. plus, even if you give the address, guests are going to need to look up directions anyhow.
i would say that if you are including a reception/direction card, then you can include the address there.
MemberPersonally, I would put the address, sans zip code, on the invite. Why make it difficult for your guests to find the address? Or, make them search for it either on another card or online, etc?

MemberYou dont want to just put the name of the church, some churches have the same name and people might go to the wrong one. This probably never happened before but there is nothing wrong with putting the address
