You can make a separate card for the reception info.  Your invitation can just give the information for the ceremony (time, location) and put the location and time of the reception on a separate card to stick in the envelope with the invitations.

I am enclosing a separate reception card.

If you don’t want to do that, then I would definitely include both addresses on the invitation…ie:

Cake and punch (or Dinner and Dancing) immediately following

Name of reception place

Address of reception place

You probably don’t need to repeat the City/state information.

I would also include the start time of the reception so people not only know where they are expected to be but also what time.

we had a separate reception card since otherwise it looked too crowded to have both addresses

Mine were in two different locations as well, but put them on the same card. My card designer just made the font smaller on the reception part to include the address. It looked fine.
