I think, for my passport, I only had to pay for the photo.  This was back in 2008, so I can’t be sure.

Oh I just looked… It’s a $140 fee for the book & card, plus $25 processing.  I believe the card can only be used in Mexico and Canada, though.

I always get insurance for every trip I take, but I do forget to take my passport in port with me, which I really should do
