Only half of my guests initialed by their names on our RSVPs. The venue requires that each individual place card indicates their entree choice. Is it OK to just guess who got what and then have them swap dishes if I guessed wrong?

Sorry, but you’re going to have to chase them down. At least as many as you possibly can.
It should be the case that people who don’t care about following instructions also don’t care about getting the right food right away. Unfortunately, the opposite is usually true.

I had/have the same problem, I had to do a ton of chasing, it’s unfortunately important especially when you think someone might have an allergy or diabetes or food issues that you don’t know about. For instance, FI’s grandpa’s GF grew up on a chicken farm and HATES chicken, one of them wanted beef and the other wanted chicken but if I hadn’t asked I never would have known. She’s also diabetic and can’t eat our dessert.. 🙁

AAuuughhh!! My Fi and I are having the same problem. We CLEARLY stated to INITIAL entree choice. Not check one! Ugh!
