Ok so I can really speak to this situation since i’m right in the middle of it and our stories are scary similar.

I have an endometrioma on my left ovary that is 4mm. My RE does not want the cyst removed as it could further impact my egg reserve. I did one cycle of IVF antagonist in May, just like you they retrieved 10 eggs, 8 fertilized, 2 embryos, 1 PGS normal embryo. I am not happy with the result of one embryo so my RE is switching me to a microdose Lupron flare protocol but not with BCP prime, it’s with estrogen prime. From what I understand when paired with the Lupron BCP can oversurpress women who have DOR and you can end up with much less follicles like what happened to you.

My RE claims I have DOR as well since my embryo count was so low I probably have egg quality issues from the endometriosis which is what that cyst is however, she strongly urges against surgery as it can further impact my numbers, which are close to yours I have an FSH of 9.2 and an AMH of 1.2. I was always told the FSH should be below a 10 and AMH above a 1 but we are borderline. My RE said it could take a few cycles to get better eggs, they’re most likely in there but it may take longer than others.

I think you need a second opinion. Do not do this surgery before you collect your eggs. There is research everywhere that shows endometrioma surgery can further impact your egg count. Even the best surgeon would have a hard time excising it completely without taking good tissue with it. I know it’s sucks to know that. I was also told anything under a 5mm doesn’t need to be removed. I am starting the new protocol in a few weeks so I will update you on how it goes. I hope this helped a little. Good luck to you Bee. I know this shit is so hard.

@jen9999:  i’m glad to hear you aren’t doing the surgery yet. I don’t think it’s a bad idea to do the antagonist protocol as it is successful for many and allows for customization. Was there any discussion regarding the micro flare protocol? Just being curious since i’m starting mine in about a week. I have not had experience with mini ivf.
