Order it now! Too many "what ifs" if you wait. : (3 votes)

Wait until shortly before baby is born. Better chance you'll still get your dress. : (0 votes)
Wait until after baby is born. Then you'll have a better idea of your size. : (4 votes)

Three months is enough time to lose some of the baby weight.  Order your dress a size bigger than what you are now and they can alter it down if needed.

If you are looking at a medieval style, any chance the dress has (or could be made with) a lace-up bodice? Medieval/renaissance-style dresses can be a lot more forgiving than many other styles of wedding dress when it comes to size.
