You need to make this change and add the apostrophe in the proper place in guest’s:

Please list each guests name and meal choice SHOULD BE

Please list each guest’s name and meal choice

 

Also, if your parents are divorced your mother might not like to be addressed as Mrs. anymore. For example, my FI’s parents are also divorced but have the same last name and I wrote it:

Mr. Stephen Smith

Ms. Susan Smith

When listing the parents’ names, I would put the “and” after his mother’s name, not before.

Use “and” to join your name and your fiance’s. Not “to” in this context.

“Accommodations” is misspelled.

I think that 

Kaylee Nicole Smith

to

Jim Barret Jones

should be

Kaylee Nicole Smith

and

Jim Barret Jones

since all your parents are inviting.

@August252012:  It should make sense as if it were written in prose form. You wouldn’t write “Mrs. Karen Smith, Mr. Thomas Smith and Doctor Cheryl Jones, Doctor William Jones request…” you would put the conjunction before the last name in the series. I agree this is a little bit awkward. Are the parents hosting?

Same for the conjunction between your names. It’s not an etiquette issue, it’s a grammatical one. It doesn’t make sense to say “at the marriage of their children, Kaylee Nicole Smith, to Jim Barret Jones.” It makes it sound like you, Kaylee, are their children, plural!
