Since your wedding is very formal, it’s proper to put their title, first name and last name. So, Mr. John Doe, Ms. Jane Doe, Mrs. Janis Doe.

Actually, “Miss Aspasia Phipps” is a business name, not a formal social name. In social situations given names and surnames are used together only when necessary to disambiguate.

So in most cases the proper formal form is “Mr Phipps”, “Mrs Phipps”, “Ms Phipps” or “Miss Phipps”, or for sons and younger brothers and for younger sisters “Mr Aloysius” and “Miss Ariana”

I think it would be more clear if you had Title + First Name + Last Name, especially if you have a lot of family members invited. For example, if we went with just Mr./Mrs. + Last Name, then DH’s cousins that are siblings would be very confused as they have the same name if you exclude the first name. I also think people are trained to look for First Name + Last Name (because of all these years of roll call).
