I really love the seamstress I have been using, Lorraine Kimball.  Her website is http://www.kimballskreations.com/

Basically, she charges $15/hour for labor and $6 to have bra cups sewn in.  My dress was purchased as a sample size 8 and I normally wear a street size 0 and so far I’ve had 3 fittings and it looks great!  We are saving my last fitting for after the holidays.  Her original estimate for all the work on the dress was $95 + $6 (bra cups) = $101.  She altered the bodice, adjusted the skirt part so it tapers to my size (it’s an A-line dress), took up the hem, and added a bustle.  She’s very thorough and meticulous, responds very quickly to emails and calls, and also a really, really nice lady. She works out of her home.  During my last fitting she mentioned that she’s booked for the rest of this year (2010) and only taking clients for the rest of next year.

@jill2wed: My cousin used Rose DeSimone in Carlsbad, San Diego to design her gown. I went to all the meetings and she was amazing! Very professional and a first class wedding gown seamstress. She is actually a wedding gown designer, however does custom wedding dresses. She designed Jenny’s gown from scratch, did custom fittings, and made everything easy and fun. Jenny’s dress came out gorgeous and she was very happy. Her website is http://www.rosedesimone.com and her phone number is 760-730-0300. If you are seriously looking for a seamstress to design your wedding dress I wouldn’t go to anyone else. Not sure on pricing. She is not cheap, however her work is amazing and she uses silk and includes the veils and petticoats for free! Good luck with your search. PS. @sunset:  I also used Lorraine for my wedding. My alterations were not wildly complicated at the start, just hemming, bustling and adding cups to a La Sposa Fanal. A week before the wedding I’d somehow manages to lose quite a bit in the chest, so she took in the top and rehemmed since the lost inches made the dress longer.  She had the second round of alterations done in 24 hours and my dress fit beautifully. To boot, she is very sweet, it was quite affordable and I wasn’t even charged the second time.

I know she had many positive reviews on the BridalInsider website, which is where I found her, but that site has been down for a while now. (Supposedly being redesigned?)
