What did you or are you going to register for?
I’ve been researching what a baby needs vs. all the bazillions of baby items on the market and hope to stick to the needs as best as I can.
I want to register for:
Changing table pad
We plan to get the crib used, as well as any swings or things like that.

We haven’t begun that process, yikes!! We are due April 21, 2013 and intend to register…I better get to it!!
In addition to what youve posted I’ll probably register for a:
Pack and Play
These are starters because I’m SURE I’m missing a ton!!

Are you nursing or bottle feeding? Cloth diapering or using disposables?
It seems like such a “luxury item”, but I actually used the crap out of my baby carrier. I would research that and register for one.

I just want to avoid spending money on items we won’t really use (wipe warmers, cutesy little washcloths, swings, etc.) and I don’t want to put them on a registry either. If we wind up using a swing, it’d be a $15 one from Craigslist because baby will grow out of it so quickly!
I was looking at my SIL’s registry and saw so many items I doubt she’ll wind up using and I want to steer clear of that, my house is too cluttered as is!

I registered this weekend and some of it is definitely not neccessities but things I really want (ie. Nursing cover, Moby carrier or Sling, boppy, Shopping Cart/High chair cover)
things you may need to register for: crib sheets! (more than one in case they have accidents) crib matress pad/cover, recieving blankets, Diaper Genie if you are using disposable diapers. Diaper bag?

In addition to what everybody else has suggested (or to piggyback on that), I’d add:
Plain burp cloths
Snap-n-go stroller (put the car seat in and go; gives you time to decide what you want in a stroller and shop used)
Hygiene and medical stuff (nail clipper kit, thermometer, hand sanitizer, etc.)
Covers for your changing pad and portable, washable waterproof changing pads for your diaper bag
Plastic dishwasher baskets for washing bottle components and/or pumping equipment
Drying rack for the above (I have and love the Munchkin one that looks like a tree)
Diaper Genie refills! (if you’re using disposables)
I fully support the suggestion to register for your crib bedding and spares. The bottles are a little tough, because you don’t always know what your baby will like, but it might be worth registering for a few different kinds to avoid spending a fortune later on.
