i think she should write a note on the bottom saying that she will be unable to attend.

That seems strange to have a response card with no option to decline!  I would have someone else take a quick look at it. Sometimes on cards with different formats, it is easy to miss something.  A fresh pair of eyes might see another way.  Is the box big enough to write ‘no’ or will not attend’ instead of a check mark? If not, I’d just leave the box blank and write a short line at the bottom.

Maybe it’s one of those that says 

Will _____ attend

where you fill in a blank?

Maybe put a 0 in the blank for attending and a not saying how sorry she’ll be to miss it?

Yes.  Sounds to me like the bride forgot to put a space for non-attending guests.  She should mail it back with her regrets.
