Mbs08:  I would just leave off the ‘names of attendees’ part. We had similar wording but left the ‘Names(s)___________________’ part blank, people filled it in and checked accept or decline. For people who were coming without a date or their kids or whatever they just wrote in the names of the people who were coming and anyone not listed we knew wasn’t attending.

That sentence is really long and I hope it makes sense.

Mbs08:  There is no foolproof way. The fools will always find a way.

You could try this:

We have reserved __ seat(s) in your honor

Name____________  Accepts ( )   Declines( )

Name___________    Accepts ( )   Declines ( )

Your updated version looks good. It’s similar to what we’re doing.
