Do you have a more neutral, blank card that you could send them?  That way, you could write a nice thank you note for the gift, and also write a nice note letting them know that you’re thinking about the wife and wish her well in her recovery.  In that way, it comes across as just a nice note vs. just being a thank you card.

I would send a seperate Get Well card at the same time.  You really don’t want to mix the two feelings but you do want to acknowledge her being ill.  Sorry your family friend is ill, I will keep her in our prayers tonight.

I think it’s absolutely appropriate to say something like “Thanks for coming to the wedding/gift. I hope you’re feeling ok, we’re thinking of you” in the same card. You should be personalizing the thank you notes anyway. I guarantee they’ll be grateful, not judging you or thinking it’s weird.

I actually did this – one of DH’s uncles fell right before the wedding and broke his leg – so he couldn’t make it.

Before the bridal shower, another uncle found out he had cancer (caught very early and is doing well now), and his wife had attended the shower.

In both cards I mentioned the uncle  (“we missed seeing Uncle X at the wedding and hope is recovering quickly…..”keeping Uncle Y in our thoughts”)

So I think it’s completely fine to thank them for their gift and take a couple lines to tell them you’re thinking of them and wishing for a speedy recovery.

I think people appreciate knowing that they are in your thoughts regardless of what type of card you use to convey your message. A simple “thank you for your gift blah blah  blah and we are keeping you in our thoughts and wish you a speedy recovery” would be well received.
