No need to be mean to your future sister. For the dress, it would be best to go with an empire waist so that it can fit her belly easily. Above all, don’t offend your in-laws. No one will be loosing focus to her on your wedding day. Even if not in your bridal party, she will be attending the wedding. If some old coot is going to gossip at your wedding, she will gossip weather your future sister is in your bridal party or not.

Don’t kick her out. Be understanding and let her know that you still want her in your bridal party but you understand if it’s too much for her to deal with. If she chooses to stay in your bridal party, choose a flowy dress and provide her with a chair during the ceremony.
