I had mine custom made using 5 different dresses as inspiration.  I needed a covered back but this was my main inspiration:

The custom gown had a different top, crossover style intead, the back was made into a lace up instead of zipper and the waist instead of flowing asymetrically was straight across.

Mine is in the process of being made by Jasmine’s Bridal Shop.
I used a White by Vera Wang (VW351124) and Pronovias Dogma for inspiration.

the VW photoshopped to have a slight sweetheart:

The Pronovias (I never did try it on! It was from the new 2013 line):

I took the organza, the inverted pleats, and the floral applique placement from the VW, and I took the lace underlay from the Pronovias Dogma.
I asked for my own customizations–a slight sweetheart neckline, a v-back, a ribbon sash, longer train, and a button back. Here’s the sketch I drew:

This is the dress in the middle of production! Still a long way to go until it’s the dress I’ve been hoping for!

This is the original dress:

Then I removed the lfower, added a belt and added a crinoline, but it wasn’t poofy enough at the hip:

So then I added more crinoline at the hips which meant that the fabric needed to be let out, and added:

Then I added a lace top!

And the final product!:
