I’m not a native speaker BUT did notice a couple things you may want to double check:

Kindly Respond by / Por favor responde antes de

___ Seats reserved in your honor/ Asientos se han reservado en su honor

___ gladly accepts / acepto con placer

___ regretfully declines /  lamento no poder aceptar

# ____ number of guest(s)/ número de atienda

Name(s) of Guest(s)/ Nombre(s) de persona(s):

I agree with the ‘your honor’.  I don’t know Spanish well though.

I agree with drivers430, also…

responde should be respondan

perosnas should be personas

I actually like “personas que asistiran” – it’s more formal. Don’t forget the accent on the last “a” though. And when doing a date it will de “responde antes del 3 de junio” or whatever.  “DEL” not “de”. And most people don’t realize that months ARE capitalized in English and are NOT capitalized in Spanish.

The correct word for “guest” would be “invitado”, not atienda. I would say “Nombre(s) de invitado(s)”.

___ regretfully declines /  lamento no poder asistir

# ____ number of guest(s)/ personas que asistiran

Respondan antes   del   fecha   de   mes  de 2010   (agree with Danielle Zara)

Seats reserved in your honor  –  Asientos reservados en su honor (drop the han)

Glady  accepts                      –  acept con placer (good)

                                          – lamento  no poder asistir  (good )

# of  guests   –  Numero (accent mark on the U) de PERSONAS  que asistiraN (accent mark on the last a )

Below is another option that you might think about:

____ podemos  ____ no podemos

aceptar su amable invitación para la Boda
