Thoughts on assited hatching with FET

We did assisted hatching with our 5 day embryo (standard at our clinic) and I’m 9 weeks with a singleton. I asked about the increased risk of twins with AH and the doctor told me that its incredibly small – not enough for him to worry about it.

Mine also stores AH for FETs. They said that frozen embryos may have a tougher shell, so they do AH to help.  I did ICSI and AH, and no twins here.

candy08 :  The term assisted hatching simply refers to the process of creating a small hole in the zona pellucida by weakening the outer shell by use of a laser or acid solution. This is done with EVERY frozen embryo – a small hole is created to drain the excess water within the cells to prevent small crystals from forming during the vitrification process. If crystals form, you have a higher risk of the embryo bursting or having an unsuccessful thaw.

When people hear the term assisted hatching, they typically are referring to this process being done with a fresh embryo transfer. The thought here is that the small “crack” in the outer shell will help the embryo to break out of the shell and implant.

Additionally, anyone who does PGD/PGS uses assisted hatching to retrieve tissue during the biopsy.

My RE explained it that every FET uses assisted hatching, because of the process to freeze there will always be a small hole or crack in your embryo. It doesn’t significantly increase your risk of multiples or success rates when done this way.

candy08 :  Are they recommending additional hatching be done once your embryo has thawed? If so maybe I am confused – but when our first transfer was unsuccessful, I inquired about ways to improve our odds (we only got 3 embyros out of our two retrieval cycles, not genetic tested). My RE explained what I wrote above, and all of the vitrification and thawing process was included in our Freeze cycle/FET.

FWIW, as I mentioned, we had only 3 embyros. I struggle with lining growth; we TTC for ~3 years with a bunch of failed IUI cycles and 2 miscarriages, one failed FET. We changed up my protocol and had a successful transfer; my son is almost 2.5years old. He had a lot of issues at birth and we were on the fence about transferring our last embryo (and never doing IVF again), but felt like we would be ok with either outcome. It took awhile to get to that place, but ultimately we felt like we owed it to everything we went through during infertility to try. I’m 17 weeks pregnant with our last embryo. I guess what I’m trying to say is not to rush things – wait until you feel confident with whatever the process may entail: success and a second child, or coming to a crossroads and either being done with IVF or moving on to another retrieval cycle. It’s hard to see that perspective, but I’m confident you will see things work out in the end. Wishing you lots of luck!

candy08 :  Thank you! That was how I felt going into this transfer, too. I knew it would be our last and wanted to make sure that I did everything I could to feel like it was in my control/I would have a successful outcome. I hope your RE can clarify the assisted hatching for you. It does seem to be a confusing topic!

candy08 :  we did ICSI too, I’m not sure if that could have played a factor. And thank you, I hope so too! There are so many more risks to worry about, especially with identical twins, but I’m hoping I’ll be able to relax a little bit. Good luck on your next transfer!

sophie89 :  I know the feeling. I keep saying ‘if we have a baby in August’, so I haven’t quite gotten over the fear that something is going to happen.
