It very clearly says:

If either of your parents has a maiden name that is different from their married name, do not include their married name. Enter their full maiden name.

So if your mothers changed their name after they were married, you enter their maiden names.

My now husband and I just got our marriage licence in Virginia about a month ago and I agree it is confusing and it took two tries to get it right, lol.

You need both your parents full names and the correct spelling of their names for your marriage licence and if you have a passport go ahead and bring that too. In Virginia the driver’s licence standards are diffrent than the passport ones. I had a problem because my middle initial in on my driver’s licence and they need your full name for a marriage licence and needed my passport to verify my middle name. The person behind the counter told me if wanted to use a middle initial I could but it might cause a problem down the line and I would have to file an affidavit to change the marriage licence info later, and you have to do it on person even if you have moved out of state.  Hope this information help.
