I would guess some really good edible painting skills on some fondant, but that’s a complete guess.

With fondant not everything has to be edible. You can just take it off. It looks like everything except the hear with lettings is removable.

I’m guessing the person baked the cake, leveled and frosted it with buttercream. Coated it with fondant. Wrapped it in some sort of metal or plastic decor and stuck in a few flowers. All of the non edible items can be removed as you cut the cake.

The letters were probably either painted on with melted chocolate or a dark food coloring. I’m going with melted chocolate because it looks raised.

It just looks like a regular fondant cake to me.. Are you trying to make it yourself or planning to have a baker replicate it?
